Has any body ever accidenty ped an OG tube into the trachea, while the pt is intubated with a cuffed ETT?

Note: The pt was being ventilated for about 12 mins. and had good pulses and b/p. CO2 read 23, and could not get a O2Sat. Dr, thought this was an esophageal intubation with placement of the OG into the trachea. Does not make sense. This was a trauma, RSI'd pt. Pt remained stable, with misting and good b/s the whole time. She had on fake nails.
